Students who participate in the athletic program at Martin Luther High School are required to have the following forms on file in the main office prior to participating in a sport:
Families are encouraged to fill out the following form and turn it into the main office incase there is an emergency medical situation that arises with the student athlete.

The W.I.A.A. (governing body of high school athletics in Wisconsin) requires that a physical exam performed by a certified doctor and recorded on the
WIAA PHYSICAL EXAM FORM be on file with the school prior to participating in a sport. The physical exam is valid for
two years. During the second year of the two-year cycle an athlete must hand in a
WIAA ALTERNATE YEAR FORM. This form makes the physical exam valid for the second year of the two-year cycle. Please see the directions below to help you determine the form your student needs on file for the
2009-2010 school year.
Incoming Freshman need to do the following:
Returning Sophomores, Juniors and Seniors must complete the following:
1. You must use the opposite form of the one you used during the 2008-2009 school year.
a.
Most Freshman and Juniors had a physical taken at the beginning of the 2008-2009 school year so they would use the
W.I.A.A. ALTERNATE YEAR FORM for their Sophomore or Senior year.
b.
Most Sophomores and Seniors did not have a physical taken at the beginning of the 2008-2009 school year so they need a physical exam and must use the
W.I.A.A. PHYSICAL EXAM FORM.
The W.I.A.A. requires parents and athletes to read and sign off on the
W.I.A.A. ATHLETIC ELIGIBILITY INFORMATION BULLETIN prior to participating in a sport. This form must be completed
each year. Click on the link to the W.I.A.A. to download the form from the W.I.A.A. website. After both parent and athlete have read the form please email to Mr. Wingfield the following:
I certify that I have read, understand, and agree to abide by all the information contained in this bulletin. I further certify that if I have not understood any information contained in this document, I have sought and received an explanation of the information prior to sending this agreement to Martin Luther High School. Include both the parent/guardian name and student-athlete name in the email.
