Hall of Honor Nomination Forms

Fill out the correct form to the best of your knowledge.  All nominations received by June 1st will be considered for the next calendar year's class.  Nominations received after June 1st will be held up to 3 years for future consideration.  The candidate’s name may be re-submitted after 3 years. 

Alumni Nomination

Criteria:

  • The nominee must be a graduate of Martin Luther. 
  • The nominee must be out of school a minimum of 10 years beyond graduation.

All information will remain confidential until nominees are actually selected.

Coach/Teacher Nomination

Criteria:

  • The coach/teacher must have been a contracted coach/teacher of Martin Luther High School. 
  • The coach/teacher cannot be currently coaching/teaching at Martin Luther High School at the time of consideration.

Friend of the Ministry Nomination

Criteria:

  • Any individual who has made significant contributions to the Martin Luther High School, in roles other than alumni or coach may be nominated.
  • A non-association employee may still be a current contributor to the Martin Luther High School.
  • The longevity of an individual's contributions is important. Contributions of time, energy, and financial support will receive consideration.
